THE PROBLEM
The bump is the symptom. The bone is the cause.
A bunion (hallux valgus) is a structural deformity of the big toe joint, not just a bump on the side of the foot. Two structural shifts happen simultaneously: the big toe drifts inward toward the second toe, while the long bone behind it (the first metatarsal) angles outward toward the inside of the foot. The visible bump is actually the head of that displaced metatarsal pressing against the skin.
Bunions are progressive — they tend to worsen over time without intervention. Genetics play the largest role, but footwear choices, foot structure, and certain conditions accelerate development. Most patients live with bunions for years before seeking care, often after the secondary problems (hammertoes, calluses, gait changes) have started to develop.
COMMON SYMPTOMS WE LOOK FOR
— Visible bump. A bony bump on the inside of the foot at the base of the big toe, often growing over time.
— Big toe drift. The big toe angling inward toward the second toe — sometimes overlapping or underlapping it.
— Pain or aching. Discomfort with weight-bearing, especially in shoes that compress the bunion area.
— Limited shoe options. Difficulty finding shoes that fit comfortably as the deformity progresses.
How We Treat It
Bunion treatment matches the stage of the deformity. Early-stage bunions respond well to conservative care: footwear modification, custom orthotics, padding, toe spacers, and addressing the underlying biomechanics that drive progression. These interventions can dramatically slow worsening and reduce daily pain.
Custom orthotics are particularly impactful for patients whose bunions are tied to flat feet, overpronation, or other biomechanical issues. Properly designed orthotics redistribute pressure away from the bunion and address the gait patterns that contribute to deformity progression.

Lapiplasty corrects bunions in three dimensions — front-to-back, side-to-side, and rotationally — while traditional bunion surgery only corrects in two dimensions. The 3D approach addresses the actual root cause of the bunion deformity, leading to lower recurrence rates and earlier weight-bearing during recovery.
Traditional osteotomies cut the metatarsal bone and shift it into a more aligned position, then hold it with screws or pins. The procedure is well-established but doesn't correct the underlying rotation of the bone, which is why bunions sometimes return years later. Lapiplasty uses precision instruments and titanium plate fixation to lock in three-dimensional anatomical correction. Both procedures have their place — we recommend the technique that produces the best outcome for your specific bunion structure.

Lapiplasty patients typically walk in a surgical boot within days of surgery, transition to normal shoes around 6 to 8 weeks, and resume full athletic activity at 4 to 6 months. Traditional bunion surgery often requires 6 to 8 weeks of non-weight-bearing before walking is allowed.
